This includes patient follow-up in the form of medication management or even going through the after-visit summary with the patient to help them understand their treatment plan. A variety of the usual services you now offer are included in PCM activities, such as follow-up following hospital visits, referral coordination, management of care transitions, revision of care plans, and assistance with receiving preventative services. Many clinics, including particular oncology clinics, are looking at ways to fill the gap in reimbursement payments when the Oncology Care Model (OCM) sunsets in June 2022.
